Bridging the Gap: Hospital to Home and the Housing Imperative
Across Western Australia, too many people with disability, mental health needs, and other vulnerabilities remain in hospital long after they are medically ready for discharge.
​
The lack of suitable housing — particularly Specialist Disability Accommodation (SDA) and community-based supported housing — creates a costly bottleneck for the health system and a stressful, disempowering experience for individuals.
This session explores the systemic challenges of transitioning from hospital to home, including housing shortages, NDIS and SDA delays, cross-sector coordination gaps, and regional inequities.
It will highlight innovative models emerging in WA and nationally, such as transitional supported housing, integrated hospital discharge planning, and new approaches to SDA development.
Attendees will gain insights into how health, housing, disability, and community sectors can collaborate to create sustainable pathways from hospital to home, reducing pressure on hospitals while delivering dignity, choice, and independence for vulnerable Western Australians.
