Tracy Patriarca is a highly experienced senior disability and community services professional with over 25 years’ expertise across the NDIS, specialist support coordination, SDA, SIL, and complex service systems.
Known for her strong advocacy, trauma-informed approach, and person-centred practice, Tracy excels in leading teams to deliver safe, high-quality outcomes for participants and families.
Her extensive experience spans navigating complex NDIS pathways, housing transitions, hospital discharge planning, crisis response, workforce supervision, and multi-stakeholder collaboration.
Tracy is particularly skilled in managing complex, high-risk cases, safeguarding, and supporting participants through statutory and guardianship processes while fostering autonomy and choice.
She is the Hospital Engagement and Sector Development Lead for NDISDA, focusing on SDA housing and disability.
In this role, she provides specialist advice, sector training, and Hospital-to-Home transition planning, supporting pathways to Specialist Disability Accommodation (SDA) or Supported Independent Living (SIL) housing for members
