From Ward to Home: Learn & Network – Hospital-to-SDA Transitions

​

This Learn & Network event brings together Supported Independent Living  providers, Support Coordinators, and

Occupational Therapists for a collaborative, solution-focused afternoon.

Attendees will explore the realities of hospital discharge into Specialist Disability Accommodation  and Community-based supports, gaining insight into system pressures, funding constraints, and coordination challenges that affect timely, safe transitions.

Through discussion, peer-to-peer sharing, and practical examples, attendees will learn strategies to navigate hospital-to-home pathways, strengthen cross-sector communication, and reduce bottlenecks that can delay participant access to the supports they need. Aligned with NDIS hospital discharge frameworks, the session emphasizes early planning, integrated stakeholder collaboration, and practical, participant-centred solutions.

The session includes:

  • 1-Hour Information Session which includes Audience Q & A and engagement : Targeted insights into 2026 hospital discharge pressures, funding constraints, and evidence requirements impacting OTs, Support Coordinators, and hospital social workers.

  • Networking & Collaboration: Structured opportunities to meet peers, share experiences, and discuss practical solutions to hospital-to-home challenges.

  • SDA Walkthrough: Experience the Alfred Cove Utano Care Apartments first hand, learning how design features support safe, independent discharge and better participant outcomes. Meet the Participants onsite 

  • Refreshments: Enjoy beverages and canapés while connecting with professionals across disciplines, fostering meaningful conversations that extend beyond the session.
