Tracy Patriarca is a highly experienced senior disability and community services professional with over 25 years’ expertise across the NDIS, Specialist Disability Accommodation,
Supported Independent Living , and complex service systems.
Known for her strong advocacy, trauma-informed approach, and person-centred practice, Tracy excels in leading teams to deliver safe, high-quality outcomes for participants and families.
She currently serves as Hospital Engagement and Sector Development Lead for NDISDA SDA Housing and Disability and Impact Housing National Strategic Alliance, driving cross-sector collaboration, hospital-to-home transition initiatives, and innovative SDA solutions.
Her extensive experience spans navigating complex NDIS pathways, housing transitions, hospital discharge planning, crisis response, workforce supervision, and multi-stakeholder collaboration.
